Facility Placement

Sometimes it is more appropriate to move to a senior facility, where varying levels of services are available. Companionship, health care services, and meals are provided as well as appropriate levels of assistance.

Care Management

Our Care Management (what many call case management), is handled carefully by our Care Manager who will travel to you, whether you’re at home or at the hospital, so we can sit with you and go over your Plan of Care. After assessing your needs, your Care Manager will help determine what type of care is appropriate, either provide in-home care services or select specific facilities to meet your health care needs.

Upon making your choice, your Care Manager will help facilitate the transition every step of the way, coordinating closely with you, your family, and either your new Caregiver or Care Facility. Your Care Manager will monitor care as your advocate and will help resolve any issues that arise.

We provide free Care Management to every home health client who receives In-Home Care Services from our A1 Care Family.
