What does a Care Manager/Case Manager do? A1care’s CM will sit with you and go over your Plan of Care. After assessing your needs, your CM will help determine what type of care is appropriate, either provide in-home care, or select specific facilities to meet your health care needs. After choosing a facility or in-home care, your CM will help facilitate the transition and will be involved with the coordination between family and client. Your CM will monitor care as your advocate and will help resolve any problems that arise.
